Physical Therapist - Part Time - Week days, Day time hours. Provides in-home evaluation, direction, and application of Physical Therapy services to relieve pain, develop or restore function, and/or maintain maximum physical performance of patient. Current Physical Therapist license or registration as required by the state is required with 1 year physical therapy experience preferred.
Physical Therapist - Part Time/PRN - Day time hours, Weekends. Provides in-home evaluation, direction, and application of Physical Therapy services to relieve pain, develop or restore function, and/or maintain maximum physical performance of patient. Responsibilities:
- Provides in-home evaluation, direction, and application of therapy services
- Observes, records, and reports reaction to treatment and any changes in patient's condition to the appropriate personnel
- Provides on going instruction, counseling and supervision of patient, family and other health team personnel in the Plan of Care and methods of assisting the patient
